Beginning January 1, 2010, all of the voluntary dental plans offered by the WELS Benefit Plans Office will be provided by Assurant Employee Benefits. Members of the WELS VEBA Group Health Care Plan are able to choose between the PPO Choice+ plan and the Freedom Schedule plan. If you live in one of the states listed separately below, the plans available are the Freedom Schedule plan and the Dental Management Organization plan offered by Assurant in your particular state.
| 2012 Dental Open Enrollment |
- The open enrollment for dental coverage effective January 1, 2012 will be held during the month of November 2011
- Eligible WELS VEBA members may enroll for new coverage or change existing coverage using the election website at www.wels.bswift.com or by calling bswift at 1-800-487-8322
- The 2012 dental plan options and rates will remain unchanged from 2011. Plan booklets are available below and the monthly premiums are available by clicking this link
