Eligibility
Basic Group Term Life
Workers (and their spouses) who are enrolled in the WELS VEBA Group Health Care Plan for active workers are automatically enrolled in Basic Group Term Life (GTL).
Optional Group Term Life
Employee Eligibility
In order to be eligible for the optional GTL coverage, a worker must meet all of the following criteria:
- Be an active worker in an eligible sponsoring organization
- Be hired or called to work at least 20 hours per week and for five or more consecutive months of the year
- Must either:
- Be enrolled for health benefits under WELS VEBA
-or-
- At least one worker at the worker's sponsoring organization must be enrolled for health benefits under WELS VEBA
Important: If WELS VEBA coverage for all the workers of a sponsoring organization is terminated, the GTL coverage for all workers will also be terminated.
Dependent Eligibility
Qualified dependents include spouses and unmarried dependent children under age 19. If a dependent child is a full-time student, eligibility may be extended to age 26. Children that cease to be full-time students or become married during this period will be terminated at the end of the quarter in which their qualifying event occurs.
Enrollment
Basic Group Term Life
Enrollment in Basic GTL is automatic for workers (and their spouses) enrolled in the WELS VEBA plan for active workers at the time of their enrollment in WELS VEBA. No additional enrollment forms or paperwork is required.
Optional Group Term Life
Open Enrollment
New workers in WELS who meet the above eligibility rules are eligible to apply for Optional GTL coverage of up to $25,000/employee and $12,500/spouse without providing evidence of insurability if the worker returns a completed open enrollment form postmarked by the 60th day of their employment start date.
Standard Enrollment
For those workers who meet the above eligibility rules and are beyond their 60th day of employment or wish to apply for Optional GTL coverage greater than $25,000/employee and $12,500/spouse, simply download, print and complete the standard enrollment form and mail the form back to the Benefit Plans Office at:

These forms are also available from the Benefit Plans Office and will be mailed upon your request.
Once the Benefit Plans Office has received the forms, they will be forwarded to Sun Life for underwriting. Sun Life will then notify the applicant of their decision. If approved, the Benefit Plans Office will bill accordingly.
