The rates for Basic Group Term Life (GTL) benefits are included with WELS VEBA health plan premiums.
The rates for the Optional GTL benefits are based on two variables: 1) the applicant's age, and 2) the amount of coverage that the applicant requests.
Below is a link to the complete rate list for employee, spouse, and dependent coverage.
Please note: The total yearly premium for dependent child(ren) coverage is equal to $7.32 regardless of the number of dependent children covered.
