Illustrations of the current WELS VEBA ID cards for the 2008-2010 plan years are shown below. These cards provide identification for WELS VEBA members for all medical services and prescription drug needs. Please present this card to all providers beginning January 1, 2008. After December 31, 2007, destroy all health/prescription drug plan ID cards with effective dates prior to January 1, 2008.
To determine if your provider is in the Blue Preferred Plue POS or National BlueCard PPO networks or if you wish to find the nearest in-network provider, you may visit the Find a Doctor page for detailed instructions.
To request additional ID cards, members of the active WELS VEBA plan should call the Anthem customer service center at 1-877-512-7875. Please note that this number will be the main contact for the Benefit Plans Office and should be used by all members for all claims-related inquiries beginning January 1, 2008.
