The 4 available plan options are:

• Single
• Family
• Employee + Spouse
• Employee + Child(ren)

The 4 available deductible options are:

• Plan 1: $500 individual / $1000 family
• Plan 2: $1000 individual / $2000 family
• Plan 3: $2400 individual / $4000 family
• Plan 4: $3000 individual / $6000 family
