Q2: What plan options are offered under the WELS VEBA Group Health Care Plan?
A2: The four available plan options are: Single, Family, Employee + Spouse, and Employee + Child(ren). The four available individual deductibles are: $500, $1000, $2400, and $3000. Family deductible amounts are double individual deductible amounts. Further details regarding available plan options can be found on this page.

Q4: How do I make a change to my deductible?
A4: You are eligible to make a change to your deductible at two different times: 1. If you take a new call or position at a new Sponsoring Organization, or 2. During the Plan's Open Election Period. If you take a new call or position, a deductible option form must be completed and postmarked within the first 60 days of your start date.
Q5: How do I add a dependent to my coverage?
A5: A dependent can be added to your coverage after the following events: Marriage, birth, adoption, placement for adoption, or loss of other coverage (including Medicaid or Children's Health Insurance Program {CHIP}). A WELS VEBA option form must be completed and postmarked within 60 days of the event if you wish to add a dependent to your coverage.
Q6: Is there a separate drug card that I need to use for prescription drugs?
A6: No, the prescription drug and medical ID cards have now been combined into one card effective January 1, 2006 with the switch in pharmacy benefit managers to Medco Health Solutions. The current medical and prescription drug cards are shown below:

A7: To request a new WELS VEBA ID Card, please contact Anthem customer service at 1-877-512-7875.
Q8: Where can I find out if my provider is in-network?
A8: To find out if your provider is in the Blue Cross Blue Shield Blue Preferred Plus POS or National BlueCard PPO Network, please visit the Find a Doctor page for detailed instructions.
Q9: How do I notify the Plan of an address change?
A9: To notify the Plan of an address change, please call our office at (414) 256-3860 or e-mail at bpo@sab.wels.net.
Q10: What do I do if I take a new call or position at a new Sponsoring Organization?
A10: Please contact the Benefit Plans Office at (414) 256-3860 or e-mail to bpo@sab.wels.net. The Benefit Plans Office will change the Sponsoring Organization to which premiums are billed (to become effective only on the first day of the month). You will also have the oportunity to make a change to your deductible if a deductible option form is completed and postmarked within 60 days of your start date.
Q11: Where should I send payment for my WELS VEBA premiums?
A11: Payment of WELS VEBA premiums should be made by check only. Checks should be made payable to WELS and should be mailed with the bottom third of the premium invoice to the following address:
