Eligibility and Enrollment

 
 

Eligibility

In order to be eligible for the Long Term Disability (LTD) coverage, a worker must meet all of the following criteria:

  • Be an active worker in an eligible sponsoring organization
  • Be hired or called to work at least 20 hours per week and for five or more consecutive months per year
  • Must either:
    • Be enrolled in the WELS VEBA Group Health Care Plan
    • -or-
    • At least one worker at the worker's sponsoring organization must be enrolled in the WELS VEBA Group Health Care Plan

Please note: A dependent will not automatically receive LTD benefits. Spouses who are workers in WELS and are eligible for LTD coverage may purchase their own individual Optional LTD coverage.


Enrollment

Enrollment in the LTD plan is automatic when a worker enrolls for health benefits under WELS VEBA. If a worker meets the eligibility rules listed above but is not enrolled in WELS VEBA, the worker may purchase the same coverage. An eligible worker may apply for Optional LTD coverage at the time of employment or at any time if they provide Evidence of Insurability.

Optional LTD Short Form Enrollment

New workers in WELS who are eligible for LTD coverage and are not enrolled for health benefits under WELS VEBA may obtain Optional LTD coverage without providing Evidence of Insurability if the worker returns a completed application form postmarked by the 60th day of their employment start date. This enrollment form can be found here: Short Enrollment Form. Once the form has been downloaded, print and complete the forms, and mail to the Benefit Plans Office at:

These forms are also available from the Benefit Plans Office and will be mailed upon your request.

Coverage will begin on the later of the worker's employment effective date or the date after postmark of the enrollment form.

Optional LTD Long Form Enrollment

If the worker is beyond the 60th day of employment, a Sun Life Evidence of Insurability form will need to be completed in addition to the enrollment form. The enrollment form including the Evidence of Insurability form can be found here: Long Enrollment Form. Once the forms have been downloaded, print and complete the forms, and mail to the Benefit Plans Office at:

These forms are also available from the Benefit Plans Office and will be mailed upon your request.

Completed enrollment forms will be forwarded to Sun Life for underwriting. Sun Life will then notify the applicant of their decision.
