Illustrations of the current WELS VEBA Anthem SmartValue ID cards 2009-2010 plan years are shown below. This card will provide identification for Anthem SmartValue plan members for all medical services and prescription drug needs. Although you should not present your red, white and blue Medicare card to providers, please be certain to retain your Medicare card for your personal records.
To request additional ID cards, members of the Anthem SmartValue plan should call Anthem customer service at 1-877-326-2201.
