Find A Doctor

For members living in Wisconsin who are searching for Wisconsin providers:
You may find a Blue Preferred Plus POS provider by using one of three methods: Go to www.anthem.com. Follow the directions below. Call the national toll-free Blue Cross Blue Shield provider telephone directory at 1-800-810-BLUE (2583). Ask your doctor if he/she is a participating provider in the Blue Preferred Plus POS network.

Go to www.anthem.com. This link will take you to the Anthem website in a new browser window.
Click the "Find a Doctor" section on the right side of the page, and then click the "Go" button that drops down beneath "Find a Doctor."
Another page will open in a new browser window. Select "Yes" if asked to display the nonsecure items. Click the "Wisconsin" radio button in the top section labeled "Anthem Blue Cross Blue Shield Directories" and click the "Next" button at the bottom of the page.
Click the "Search by Location" tab at the top if you want to search for all providers near a certain location. Click the "Lookup by Name" tab at the top if you want to search for a specific provider by name.
In the "Select a Plan" drop-down menu, select "Blue Preferred Plus (POS) ". Complete the provider type, specialty category and/or name information. Click the "Next" button at the bottom of the page.
Complete the address information and click the "View Results" button at the bottom of the page.
The results of your search will now appear.

For WELS VEBA members searching for providers outside Wisconsin:
You may find a National BlueCard PPO provider by using one of three methods: Go to www.anthem.com. Follow the directions below. Call the national toll-free Blue Cross Blue Shield provider telephone directory at 1-800-810-BLUE (2583). Ask your doctor if he/she is a participating provider in the Blue Cross Blue Shield National BlueCard PPO network.

Go to www.anthem.com. This link will take you to the Anthem website in a new browser window.
Click the "Find a Doctor" section on the right side of the page, and then click the "Go" button that drops down beneath "Find a Doctor."
Another page will open in a new browser window. Select "Yes" if asked to display the nonsecure items. Click the "Search the National BlueCard Directory" radio button in the bottom section labeled "National Directories" and click the "Next" button at the bottom of the page.
In the left column labeled "Members," enter the three-digit prefix listed on your ID card, which is DQT for members not living in Wisconsin. The click the "Next" button.
Click the "Search by Location" tab at the top if you want to search for all providers near a certain location. Enter the location information and click the "Next" button at the bottom of the page. Then complete the provider type and specialty category on the following page and click "View Results."
Click the "Lookup by Name" tab at the top if you want to search for a specific provider by name. Enter the name and location information and click the "View Results" button at the bottom of the page.
The results of your search will now appear.
