You may find a Blue Preferred Plus POS provider by using one of three methods:
  1. Go to www.anthem.com. Follow the directions below.
  2. Call the national toll-free Blue Cross Blue Shield provider telephone directory at 1-800-810-BLUE (2583).
  3. Ask your doctor if he/she is a participating provider in the Blue Preferred Plus POS network.


Blue Preferred Plus POS Online Provider Finder directions:
  1. Go to www.anthem.com. This link will take you to the Anthem website in a new browser window.
  2. Click on the "Members" tab in the upper-left corner. Then click "National Accounts" in the drop-down menu to the right and click the "Enter" button. This will open a new page.
  3. If you wish to login or register for access to your personalized MyAnthem web account, you may do so on this page. If you simply wish to find a provider, click "Search the Provider Directory" in the middle of the page.
  4. Select "Yes" if asked to display the nonsecure items. In the left column labeled "Members", enter the three-digit prefix listed on your ID card, which is DQX for Wisconsin members. The click the "Next" button. This will open a new browser window. Select "Continue" in this window to resume your search.
  5. Click the "Wisconsin" radio button in the top section labeled "Anthem Blue Cross Blue Shield Directories" and click the "Next" button at the bottom of the page.
  6. Click the "Search by Location" tab at the top if you want to search for all providers near a certain location. Click the "Lookup by Name" tab at the top if you want to search for a specific provider by name.
  7. In the "Select a Plan" drop-down menu, select "Blue Preferred Plus (POS) ". Complete the provider type, specialty category and/or name information. Click the "Next" button at the bottom of the page.
  8. Complete the address information and click the "View Results" button at the bottom of the page.
  9. The results of your search will now appear.

You may find a National BlueCard PPO provider by using one of three methods:
